Forms
All members of the University community are strongly encouraged to report suspected violations of the policy; further, it is strongly recommended that instructors discuss their concerns with the student.
Members of the University community other than the instructor who suspect that a student has violated the policy may report their concerns to the school/college academic integrity coordinator or AIO. (See Procedures, section II.D.)
Students may appeal the violation within seven business days by filling out the Student Request for Hearing form (MS Word). A school/college hearing panel decision may be appealed by filling out an Appeal Form for University Level Appeal or Review (MS Word).
Instructor Forms
To report a violation, send a Violation Report form (Instructor Resolution) (MS Word) to the Academic Integrity Coordinator in your school or college. To be in compliance with the Policy, you must submit this form within five business days after determining that there is a violation.
Instructors also have the option of requesting a hearing by a school/college hearing panel. Send a Request for Hearing form (Hearing Panel Resolution) (MS Word) within five days after determining that there is a violation.
Student Forms
Students have the right to appeal a violation by submitting the Student Request for Hearing form (MS Word) to the school/college Academic Integrity Coordinator within seven business days after notification of the violation from the AIO.
School/college Hearing Panel decisions may be appealed to a University Appeal and Review Panel within seven days of the decision under specified grounds for appeal.
